All players must complete the South Metro Football League player registration form and return to the club either by mail or in person. (click here to take you to the document). A copy of a Birth Certificate or proof of date of birth must be submitted with the registration form.

In Person: Complete and lodge the following at the Registration Session to be held on March 17th between 9.30 – 11.00am at Essex Heights Reserve.

1. SMJFL Registration form (with copy of birth certificate)

2. Parental Permission and Consent for Medical Treatment form.

3. Full payment for the registration and clothing purchases. See below for details.

2012 fees:

$160 per child

$130 for the 2nd child in same family

$100 for each additional child in same family

Parents will also need to purchase from the club:

* 2 pairs of shorts

(1 home – black & 1 away - white) @ $20.00 (=$40.00)

Please note jumpers remain the property of the club and are issued on loan to the player for use in matches during the current season only. All jumpers are required to be returned at the end of each season to the Team Manager. A player will be charged for a lost jumper.
